Can the Densitometric Measurement of Arms Reflect Axial Bone in Postmenopausal Women?

Abstract
Objective
s: To determine relationship among BMD (Bone Mass Density) values obtained from axial and appendicular bone, and to know whether the arms can reflect the bony status of axial bone in diagnosis of osteoporosis.
Methods
From December 2000 to June 2003, BMD of total femur, femur neck, lumbar, arms, and legs were measured by dual energy X-ray absorptiometry simultaneously on 169 postmenopausal women who visited the department of obstetrics and gynecology, Chung-Ang University. We classified the women into normal, osteopenia, and osteoporosis according to the World Health Organization (WHO) criteria.
Results
The prevalence of osteoporosis was 3.6%, 12.4%, 3.6% at total femur, lumbar spine, arms, respectively. The correlation coefficients among BMDs of different sites were ranging from 0.726 to 0.874. The highest degree of correlation was shown on total femur and arms, the lowest correlation was on lumbar spine and arms. Variable degrees of concordant rate were found ranging from 69.8~82.2%. the sensitivity, specificity and predictive value were high in diagnosis of osteoporosis of axial bone by the densitomertic measurement of arms.
Conclusion
In measurement of BMD to diagnose osteoporosis, the axial bone and arms showed highly correlated, so measurement of BMD at arms is a competent predictor of BMD at axial bone.
