Effect of Disodium Pamidronate on the Interface of External Fixation

Abstract |
The use of external fixators are being popular for treatment of fractures, but because of its dynamic characteristics, there is a problem of loosening between the pin and bone. Many attemps have been made to solve this problem, but mechanically there are limits. Biphosphonate is generally known as therapeutic agent of osteoporosis by preventing bone resorption. It has been reported recently that it may reduce the loosening between the implant and bone in a patient who received total joint arthroplasty. The loosening of external fixation pin is occurred by resorption of bone through activation of osteoclast cells. It is presumed that if biphosphonate's effect of preventing bone resorption can increase the strength between the pin and bone, the problem of external fixator can be supplemented. Twelve dogs were divided into 2 groups; six dogs were treated with disodium pamidronate 0.5 mg/kg every two weeks for 6 weeks and the remaing six dogs were treated with placebo. External fixator with 6 pins was applied on both tibia and osteotomy of 1 mm interval was performed on right tibia, perpendicular to the long axis of tibia for all 12 dogs. Tourque strength could not be measured in 4 cases of 72 pins in medication group and 12 cases in control group because of pin loosening due to definite infection. The pin loosening in medication group was significantly lower than control group (p<0.01). The average increment of maximal tourque strength in measureable pins were 0.35 Nm in medication group and 0.27 Nm in control group, which is statistically insignificant. Results of maximal breakage strength measured by 3 point bending test in united right tibia showed no difference between the medication and control group statistically. That means that pamidronate didn't delay the bone healing. Stress-shielded left tibia showed significantly higher ultimate strength (p<0.01).
