The Association of Oophorectomy Status and the Change of Bone Mineral Density in Hysterectomized Perimenopausal Women

Abstract
Objective
The relationship between a hysterectomy and oophorectomy according to the changes in the bone mineral density (BMD) was examined in perimenopausal women undergoing estrogen replacement therapy.
Methods
Women aged 43~51 years attended the out-patient clinic at the department of obstetrics and gynecology, Kangnam St. Marys Hospital from April 1997 to March 2002 to determine if a hysterectomy and oophorectomy is required. A spine and hip BMD was assessed with DEXA at before and approximately 2 years after surgery. The subjects were divided into three groups according to the oophorectomy status. (group 1: hysterectomy only, n=15; group 2: hysterectomy with unilateral oophorectomy, n=14; group 3: hysterectomy with bilateral oophorectomy, n=16).
Results
Preoperatively, there was a significant difference in the BMD of the spine and femur, and the estrogen level between the groups (p<0.05). However, 2 years after surgery, there were no difference according to the oophorectomy status in the BMD and % change in the BMD of the spine and femur. The interval between surgery and the start of hormone replacement therapy (HRT) in group 1 was 2.35 years, 1.25 years in group 2, and 0.67 years in group 3.
Conclusion
There was no difference in the level of bone loss by hysterectomy with or without an oophorectomy for 2 years. HRT beginning a short time after the hysterectomy or an oophorectomy can minimize the loss of bone that occurs normally during those years.
